The procedure for online application of Birth Certificate is simple and you can apply online by visiting the website of New Delhi Municipal Corporation and filling the online application form. A.      INSTITUTIONAL BIRTH
1. Application procedure
Birth application form duly filled by head of the family/Relative etc which are available at International Inoculation Centre (HQ) Mandir marg/Palika Suvidha Kendra/Other Concerned Areas Birth and Death Centres.
The forms shall be available on the NDMC Website also.
2.  Fee Structure
  • Within 21 days of the occurrence of Birth- No Fee
  • After 21 days to 30 days of the occurrence of Birth - Rs. 7/- As late fee
  • After 30 days to within 1 year of the occurrence of Birth - Rs. 10/- As late fee
  • Beyond One year of the occurrence of Birth - SDM Order from local area + Rs. 15 As late fee

Documents Required:

Following Documents are Required for Registration of Home Birth within One Year of its Occurrence:

1. Duly filled birth reporting form by head of the family or relatives.
2. Certificate of Doctor etc. who assist in delivery of child.
3. Residence Proof (Election Card, Ration Card, Driving Licence, Electric Bill etc.)

Beyond one year of occurrence of birth event the registration will be done after submission of S.D.M order from local area along with Rs. 10/- as late fee to international inoculation centre (HQ) Birth Centre Mandir Marg

 Entry of Child Name in Birth Record

Within One Year: 
Hand written application to Registrar birth having subject name inclusion of child by the concerned child parent and to be submitted at all birth centres.
No fee

After One Year: 
i) Hand written application to Registrar birth having subject name inclusion of child by the concerned child parent and to be submitted at International Inoculation Centre (HQ) Birth Centre, Mandir Marg.
ii) Affidavit stamp paper on Rs.10/- duly attested by notary public having subject name inclusion of child with supporting documents like Passport, Ration Card, Voter Identity Card, 10th class certificate etc.
Rs.5/- As late fee + Affidavit

CORRECTION IN BIRTH CERTIFICATE
