The procedure for online application of Death Certificate is very simple and you can apply online by visiting the website of New Delhi Municipal Corporation and filling the online application form. This application form mainly requires mentioned below information about applicant with self declaration for providing true information:

A.      INSTITUTIONAL DEATH 1. Application procedure Death application form duly filled by head of the family/Relative etc which are available at International Inoculation Center (HQ) Mandir marg/Palika Suvidha Kendra/Other Concerned Areas Birth and Death Centers. Application procedure  Same as for Institutional Death 2.  Fee Structure  Same as for Institutional Death 3.Where to Submit the Application  Same as for Institutional Death Documents Required Following Documents are Required for Registration of Home Death with in One Year of its Occurrence:  1. Duly filled Death reporting form by head of the family or relatives. 2. Certificate of Doctor etc. who verified cause of Death. 3. Residence Proof (Election Card,Ration Card, Driving Licence, Electric Bill etc.) 4. Receipt of Cremation Ground Beyond one year of occurrence of Death event the registration will be done after submission of S.D.M order from local area along with Rs. 15/- as late fee to international inoculation centre (HQ) Birth & Death Center, Mandir Marg 1 Within 21 days of the occurrence of Death No fee 2 After 21 days to 30 days of the occurrence of Death Rs.7/- As late fee 3 After 30 days to within 1 year of the occurrence of Death Rs.10/- As late fee 4 Beyond One year of the occurrence of Death SDM Order from local area + Rs.15/- As late fee
